Introduzione Dedico questo libro a miei studenti del corso di Medicina e Chirurgia dell’Università degli Studi di Padova. Essere il loro professore e mentore della disciplina è per me un privilegio e un onore. Il presente volume intitolato Nefrologia Medica è parte di una breve collana che mira a fornire a studenti e specializzandi un sapere conciso ma completo dei vari aspetti della nefrologia medica. Altri volumi riguardano la Dialisi Peritoneale (Piccin Nuova Libraria, © 2019), la Emodialisi extracorporea, la Nefrologia Critica e il Trapianto Renale.  In questo volume abbiamo affrontato tutti gli argomenti in modo sintetico per poter consentire agli studenti di avere un testo da consultare per l’esame universitario di nefrologia. Al tempo stesso, i contenuti presentano un misto di nosografia classica e di esperienza sul campo che potrà risultare utile e di piacevole consultazione anche per specializzandi e per medici specialisti di questa e di altre discipline.  La nefrologia medica oggi non è più solamente una branca della medicina interna, ma rappresenta un vero e proprio ramo disciplinare a sé stante. Il nefrologo moderno deve prendersi cura di pazienti anziani e con importanti cronicità, ma al tempo stesso deve affrontare urgenze ed emergenze tipiche della medicina intensiva. Basti pensare al campo della sepsi, dell’insufficienza renale acuta in ambito di disfunzioni multi-organiche e di patologie oncologiche iperacute. Le competenze devono dunque spaziare da una buona impostazione diagnostica ad una strategia terapeutica che comprenda indicazioni dietetiche, somministrazione di protocolli farmacologici sofisticati, applicazioni di tecniche extracorporee avanzate di depurazione ematica e di supporto multiorgano. Con questo spirito, il volume Nefrologia Medica rappresenta un compendio a cui riferirsi per una sintesi della materia e in cui trovare spunto per approfondimenti che dovranno condurre ad una lettura delle pubblicazioni più recenti e di testi specialistici dedicati ai vari argomenti.  La missione di un professore universitario è quella di far conoscere agli studenti la materia e di farla amare e rispettare. Ma la missione estesa è anche quella di dare dei consigli per la futura professione indirizzando i futuri medici verso una professione fatta sì di tecnologie e raffinate procedure, ma senza mai dimenticare l’impegno umanistico e il contatto empatico con il paziente. Alcuniconsigli di questo libro dunque, non sono limitati alla nefrologia come specialità, ma estendono il loro spirito in una sorta di vademecum per la professione e di scuola di vita. Come ho già scritto in passato, la vita è un dono meraviglioso. C’è chi ne fa un uso opaco, incolore ed amorfo, e chi ne fa un uso smodato, intenso ed estremo. C’è chi considera ogni giorno uguale al precedente e chi considera il sorgere di un nuovo sole una occasione di rinnovamento e di eccitazione.  C’è chi la butta via e chi se la tiene cara, ma c’è soprattutto chi gestisce quella degli altri e cerca di farlo nel modo migliore: questi è il medico. Non il laureato in medicina, ma il Medico con la M maiuscola. La scienza è sempre un lavoro di squadra e io sono fortunato e onorato di avere avuto con me una squadra eccezionale per la realizzazione di questo testo. Devo quindi un ringraziamento particolare a tutti i collaboratori che hanno permesso di completare questo volume in tempi brevissimi e che ho il piacere di ricordare come colleghi di grande valore umano e professionale.  Sono grato infine al Dottor Nicola Piccin per aver accettato di portare a compimento questo progetto con la professionalità e l’impegno caratteristici della sua casa editrice.
