La terza edizione di questo libro, tutto sommato fortunato, è praticamente un nuovo libro. Poche sono le parti delle precedenti edizioni conservate ed anch’esse sono state ampiamente integrate ed arricchite. L’opera ha richiesto, globalmente, un impegno certamente superiore rispetto alle edizioni precedenti e si presenta, rispetto ad esse, più completa e, spero, esaustiva. Ancora, come nel passato, il materiale su cui il libro si fonda è costituito da appunti manualmente raccolti nei convegni, congressi e corsi di perfezionamento citati alla fine dell’opera. Rispetto al passato, tuttavia, in alcuni punti si è fatto ricorso alle originali fonti della letteratura medica e, in più parti, in particolare laddove trovano spazio suggerimenti pratici ed indicazioni di comportamento concreto (“Trucchi del mestiere”, “Lampadine”, “Scenari clinici”), sono state riportate esperienze professionali, anche personali. Tra le novità mi sembra interessante la presenza per ogni sezione di rilievo (cute, apparato respiratorio, allergie alimentari, gastroenteropatie, test diagnostici, ecc.), di una parte dedicata alla trattazione dei diversi “scenari clinici” di presentazione. Nell’ambito di tali scenari vengono considerati anche i singoli sintomi (tosse, wheezing, stridore, vomito, diarrea, deficit di crescita, crisi pianto del piccolo lattante, ostruzione nasale, russamento, ecc.) e i criteri di diagnosi differenziale. Il testo risultante è sicuramente più ricco di contenuti rispetto alle edizioni precedenti ma, ancora, per lo meno nelle mie intenzioni, dovrebbero essere salvaguardate la chiarezza e, soprattutto, la pratica utilità. Per tale motivo, essendo stato concepito come testo di consultazione, alcune considerazioni e alcuni riferimenti sono ripetuti più volte, nella trattazione di argomenti e patologie diversi. Un difetto delle edizioni precedenti, legato al fatto stesso che trattasi di una raccolta di appunti, è la mancanza di riferimenti bibliografici. Ho cercato di ovviare a ciò inserendo, direttamente nel corso della trattazione degli argomenti, riferimenti agli studi e alle pubblicazioni, soprattutto quelli più recenti, citati nelle relazioni congressuali che sono la fonte dei miei appunti. In tal modo è reso possibile l’approfondimento, qualora desiderato. Il mio ringraziamento riconoscente va al calore ed alle sollecitazioni costanti di tanti colleghi che sono stati stimolo a perseverare in un impegno che, a momenti, è stato gravoso e condizionante. Spero di ricevere da questo nuovo libro le medesime gratificazioni che mi hanno regalato le edizioni precedenti.
