Tra le infezioni nosocomiali rilevate nelle Unità di Terapia Intensiva, le più frequenti e temute sono le polmoniti associate a ventilazione meccanica (VAP, Ventilator Associateci Pneumoniae), responsabili di elevata mortalità, morbilità, tempi prolungati di svezzamento dal ventilatore ed ampio utilizzo di terapia antibiotica. Si definisce VAP l'infezione del parenchima polmonare in pazienti intubati e ventilati meccanicamente da almeno 48 ore; Essa è infatti un'importante complicanza della ventilazione meccanica associata a esiti avversi e costi di assistenza più elevati. La mortalità attribuibile stimata di VAP è di circa il 10%, con una maggiore incidenza nei pazienti di sesso maschile, con ARDS, BPCO, lesioni cerebrali, supporto ECMO. L'identificazione dei fattori associati allo sviluppo e alla prevenzione della VAP è essenziale per medici e pazienti. La conferma microbiologica dell'infezione è data dall'identificazione del patogeno a un esame microbiologico su campioni raccolti nelle vie aeree distali tramite lavaggio broncoalveolare. Mentre la radiografia del torace ha una scarsa precisione diagnostica, l'ecografia polmonare è utile per la diagnosi precoce dell'infezione al letto del paziente e per monitorare l'efficacia della terapia antibiotica. Il trattamento deve essere selezionato tenendo conto del livello di rischio di multi-resistenze e del rischio di mortalità. Gli antibiotici devono essere descalati non appena è disponibile l'antibiogramma o sospesi se le colture risultano negative. Nonostante i progressi nella terapia antibiotica, nell'imaging e la diffusione di strategie di prevenzione, la VAP resta ad oggi un'importante causa di mortalità, prolungamento della degenza in ospedale e significativo incremento dei costi dell'assistenza. Sebbene alcuni aspetti siano ancora controversi, altri punti hanno un'importanza consolidata: sospetto clinico precoce, diagnosi microbiologica, terapia mirata e prevenzione.
