L’autore, infatti, inserisce immediatamente tre “dettami” che guidano la corretta lettura: 1) Gli aneddoti ed episodi di vita vissuta utili a far sentire meno solo il principiante. 2) I messaggi dell’autore cui prestare particolare attenzione e 3) le raccomandazioni segnalate con un simbolo particolare". Questo volume sull’emodialisi extracorporea si aggiunge alla trilogia dedicata dall’autore alla nefrologia medica.Presentazione È con grande piacere che ho accettato l’invito dell’amico Claudio Ronco di scrivere la presentazione di questo volume sull’emodialisi extracorporea che si aggiunge alla trilogia dedicata dall’autore alla nefrologia medica.  Il primo motivo è che Claudio Ronco ha interpretato nel migliore dei modi, con entusiasmo ed investimento di passione ed energia, il ruolo accademico che trova nella didattica un elemento fondamentale, ed anzi la principale ragione dell’esistenza dell’Accademia e dell’Università: la missione di trasmettere il sapere agli studenti. E non a caso, Claudio più volte ha detto e ripetuto che l’Universitàesiste perché esistono gli studenti e non i docenti, declinando il principio in atti concreti quali la preparazione accurata di lezioni non solo teoriche ma corredate ed arricchite da esercitazioni pratiche e “sul campo”, lezioni che solo i veri Maestri sanno fare. E sono lezioni che rimangono per sempre patrimonio dello studente e memoria perenne degli anni della sua formazione. Confesso che per me, e fortunatamente più volte, nella visita di Ospedali dove erano ricoverati parenti o amici, essere riconosciuto dal medico di guardia o responsabile del paziente, è stato motivo di grande soddisfazione, anche se con apprensione facevo seguito al saluto con la domanda “ma all’esame sei stato soddisfatto, ti ho dato un voto corretto?”.  Il secondo motivo, che si intreccia con il primo, è che nel volume è molto chiaro l’intento di evitare quanto scritto da RK Rathbun “una lezione è il processo grazie al quale gli appunti del professore diventano appunti dello studente senza passare per la mente di nessuno dei due”. È sufficiente leggere il primo capitolo del volume per capire che l’autore ha centrato in pieno l’obiettivo di coinvolgere lo studente ed il lettore nel progetto di appropriarsi delle conoscenze, e sono molte, contenute nel libro. L’autore, infatti, inserisce immediatamente tre “dettami” che guidano la corretta lettura: 1) Gli aneddoti ed episodi di vita vissuta utili a farsentire meno solo il principiante. 2) I messaggi dell’autore cui prestare particolare attenzione e 3) le raccomandazioni segnalate con un simbolo particolare.  Sinceramente, non credo di aver mai letto un “incipit” così appropriato ed innovativo nell’introduzione ad un volume “formativo”, i cui dettami sono coerentemente ripresi nella struttura di tutti i capitoli seguenti. Il terzo motivo, è che nel mio mandato di Presidente della Scuola di Medicina e Chirurgia dell’Università di Padova, ho cercato di ridare centralità alla didattica, ponendo rimedio ad un “sine cura” che pericolosamente si era evidenziato in anni nei quali l’esasperata valutazione della “ricerca” e della rincorsa al “publish or perish” aveva distratto molti Colleghi. La mia fortuna è stata di trovare un prorettore alla Didattica (ora Rettore dell’Università) non solo preparata ma fortemente centrata sull’obiettivo di riportare al centro della scena la didattica e ditrovare molti Colleghi, specialmente di sesso femminile, capaci di cogliere l’urgenza e la necessità del cambiamento. La didattica deve cambiare e sta cambiando perché i tempi e gli studenti sono cambiati e gli strumenti dell’insegnamento vanno riformati e migliorati. Ma, anche se è vero che “teaching for learning” è lo slogan del cambiamento, e che il “Team Based Learning”, ossia un modello di classe inversa basato sulla soluzione di problemi tramite lavoro di gruppo, e la creazione di skill-labs per affinare le abilità pratiche in un ambiente il più possibile simile alla realtà, utilizzando tecnologie digitali, manichini e skill trainers sono strumenti utilissimi, il libro rimane un elemento insostituibile.  Il libro, se scritto come in questo caso, da un autore autorevole e competente, è essenziale per evitare che gli studenti si rifugino in internet e nella lettura acritica di nozioni ed informazioni non basate “sulle prove”, se non addirittura fake news.  Nel Capitolo iniziale del volume, il primo “messaggio da portare a casa” è paradigmatico: «prima lezione per i meno esperti è quella di non aver paura di dire “non lo so” e di non aver paura di chiedere aiuto. La brutta figura la fa colui che non sa e che non fa niente per imparare. Con il tempo acquisterete sicurezza e quindi non siate in ansia. L’ansia si trasmette agli operatori e ai pazienti e porta facilmente a errori». A questo, fa immediatamente seguito il secondo “take home message”: «non ascoltate quelli che vi dicono che tutto è già stato scoperto e che ulteriori miglioramenti sono impossibili. Vi sarà sempre spazio per il contributo di un ricercatore e di uno spirito innovatore […]. I progetti che tutti sanno essere impossibili, sono tali finché non arriva uno che non lo sa e li realizza».  L’autore del libro, oltre ad essere uno dei più riconosciuti e competenti nefrologi a livello internazionale è anche un personaggio schietto e chiaro nei messaggi, come quello che appare nel Capitolo 2. «Nei libri tradizionali di medicina interna e di nefrologia, vi sono interi capitoli dedicati all’insufficienza renale cronica terminale in cui vengono descritti quadri come la brina uremica (cristallidi urea depositati sulla pelle del paziente dopo evaporazione del sudore) ed il coma uremico». Ma questo non è lo stato-dell’arte oggi e Ronco confessa:  «In vita mia non ho mai visto un caso di coma uremico e tanto meno la brina uremica sulla pelle di qualche paziente» a suggellare due elementi fondamentali. Il primo è che i progressi della medicina e la gestione moderna del paziente renale hanno modificato la storia naturale della malattia ed i quadri clinici che il medico si trova ad affrontare oggi. Fortunatamente, vorrei aggiungere.  Il secondo elemento è che questi cambiamenti, non solo giustificano, ma rendono necessari volumi come questo per dare in mano agli studenti ed ai nuovi medici il quadro attuale, e non quello di alcuni decenni o anni scorsi. La medicina si rinnova, cambia, migliora la prevenzione, la diagnosi precoce, il trattamento personalizzato del paziente ed anzi si proietta a lasciare la dimensioneesclusivamente curativa per ricercare quella della prevenzione della salute e del benessere dei singoli e dell’intera comunità.  Vorrei concludere sottolineando un ulteriore elemento: Claudio Ronco non è solo un grande clinico, ma è anche un ottimo scrittore e lo ha ampiamente dimostrato pubblicando romanzi e libri di grande successo. Come ci insegna Italo Calvino lo stile di scrittura è il “punto in cui si saldano individualità e comunicazione, contenuto etico e forma”. This book tells the extraordinary story of how the function of the first - and so far almost the only - human organ was replaced by a machine, and the "artificial kidney" entered medical and public folk-lore. A practical artificial kidney, or dialyser, came about by advances in science followed by the acquisition of new synthetic materials which made the application of these ideas possible. However it was the dedication and persistence of a number of talented pioneers who pressed ahead against professional opposition to achieve success, first in the treatment of temporary, recoverable kidney failure, and then permanent renal shut-down which made it a success. The apparent high cost and limited availability of this form of treatment immediately raised ethical questions which had never been questioned before, centering around equity of access to treatment, when and if treatment could be denied, and - worst of all - the agonising decision of when, once established, it should be stopped. Spiralling costs as the true number of people with kidney failure became evident raised major political and financial questions, which were addressed in different countries in different ways which reflected - but also helped change - patterns of how medical care is provided. In developed countries, the problem could be solved by allocating a disproportionate amount of money to the treatment of relatively few kidney patients, but in the developing world the cost of treatment still limits its availability, as it does all forms of modern health care. Nevertheless, today almost one million people world-wide are maintained alive following terminal kidney failure, two thirds of them by various forms of dialysis and the remainder bearing kidney transplants, almost always placed after a period on dialysis. The story is also the sum of the often heroic lives of these hundreds of thousands of patients, a few of whom have today been maintained alive and active for more than 35 years, and many of whom suffered known, but also unexpected complications as a result of their treatment.
