PRESENTAZIONEÈ un grande piacere per me presentare le Linee-guida SIGO sulla gravidanze ad alto rischio pubblicate a dicembre 2025.Sono il frutto di un lungo lavoro che ha coinvolto un gran numero di professionisti di diverse discipline ed associazioni di persone.È la prima volta che in Italia sono state redatte linee-guida su questa tematica, complessa e sempre più attuale. È vero che i parti diminuiscono, ma è altrettanto vero che l'età materna al primo parto aumenta, che le gravidanze in donne oltre i 40 anni sono più frequenti e che la percentuale di gravidanze ad alto rischio sta diventando una realtà che sempre più spesso dobbiamo affrontare nella nostrapratica clinica.La diade donna-feto è un unicum nella medicina: l'ostetricia è l'unica parte della medicina in cui ci si deve prendere cura di due persone in contemporanea ed ha delle caratteristiche che la rendono unica, affascinante ma anche con possibilità di imprevisti, talora imprevedibili.Donna e neonato in salute sono l'obiettivo a cui tutti tendiamo, ma non dobbiamo dimenticare che la salute delle generazioni future inizia già nella vita intra-uterina, anzi prima del concepimento.Il ruolo dei professionisti della medicina materno-fetale è essenziale non solo nella fase preconcezio-nale e nel percorso gravidanza-parto, ma per quanto ciò che avviene in questi fasi andrà ad essere un determinante di salute per il futuro.Come ben sappiamo, la medicina è in continua evoluzione e quindi l'aggiornamento e la lettura critica dei lavori pubblicati sono strumenti essenziali nella stesura di linee-guida e la loro periodica revisione ne è una caratteristica intrinseca.Grazie di cuore a Tullio che ha saputo con impegno e passione coordinare il gruppo di lavoro e portare a termine questo impegno gravoso e non sempre facile, grazie a tutti i componenti che hanno dato il loro contributo ad un documento che potrà essere di aiuto nella attività clinica per continuare a migliorare la qualità dell'assistenza alle donne ed ai feti/neonati.Spero che queste linee-guida possano davvero rappresentare un mattone utile nel costruire una ostetricia sempre più attenta ai bisogni di tutti: medici, ostetriche, personale sanitario, donne e coppie.Solo tutti insieme, seppure da punti di vista diversi, possiamo migliorare la qualità dell'assistenza!Elsa VioraPresidente Sigo
