Un ritorno alla vita normale più veloce di prima.Il detto “grande incisione, grande chirurgo” non è più Vangelo.Dalle sue premesse ad oggi, la laparoscopia è diventata la via preferita dai chirurghi addestrati per molteplici indicazioni, la cui lista, non esaustiva, si allunga nel corso degli anni, grazie all’esperienza, alle idee dei chirurghi più ingegnosi e alla condivisione di questa conoscenza al servizio di altri.La chirurgia è una specialità medica che si definisce etimologicamente con la pratica della cura attraverso l’uso delle mani. Si tratta quindi di intervenire fisicamente sui tessuti, attraverso incisioni e suture.La chirurgia, come sopra definita, esiste dai tempi più antichi, poiché le tracce dei primi interventi chirurgici risalgono all’era preistorica, dove i nostri antenati già compivano trapassi, incisioni.Nel corso dei secoli e nel corso della storia, l’uomo ha sempre messo la sua mente e la sua creatività al servizio dell’innovazione. La medicina e la chirurgia ne beneficiarono ampiamente.Per passare da barbieri a medici riconosciuti, i primi chirurghi dovettero aspettare la Rivoluzione Francese. La riunificazione delle professioni mediche e chirurgiche offrì ai chirurghi il diritto di accedere all’Insegnamento ufficiale universitario e alla ricerca. Ciò rese possibile il miglioramento delle conoscenze anatomiche e fisiologiche, nonché delle regole di asepsi.Così nacque la chirurgia moderna.Nel 1944, un ginecologo francese, il Professor Raoul Palmer, ebbe l’idea geniale di introdurre un’ottica illuminata nella cavità peritoneale di una paziente, a scopo diagnostico. Da una scintilla di creatività apparve timidamente il concetto maestro della laparoscopia, che avrebbe cambiato per sempre la chirurgia addominale e digestiva, e tante altre specialità.Nel 1972, i Dottori Bruat e Manhes effettuarono il primo trattamento conservativo di una gravidanza extrauterina all’Hotel Dieu di Clermond Ferrand.Nel 1987, il Professor Philippe Mouret realizzava la prima colecistectomia mediante laparoscopia alla Clinique de la Sauvegarde di Lione.Immaginate lo sconvolgimento di un’intera comunità, che fremette all’idea di imparare e sviluppare la chirurgia di domani.Laparoscopia significa garantire ai pazienti un trattamento meno aggressivo, potendo promettergli meno infezioni, meno complicazioni parietali, meno dolore post-operatorio. Senza dimenticare i nostri numerosi colleghi di altre specialità che ormai operano anche attraverso la chirurgia videoassistita patologie toraciche, articolari, urologiche…Oggi ci sono in tutto il mondo centri esperti in chirurgia laparoscopica. Migliaia di chirurghi di tutte le età, di tutti i generi, di tutte le origini, lavorano quotidianamente per apprenderne le basi e perfezionare la propria capacità tecnica. In open surgery, only the two first - sistants can precisely capture by direct vision what is happ- ing in the depth of the operating field. Therefore, the duration of apprenticeship is long and restricted to a small number of people per teacher. The introduction of movie cameras into the operating rooms improved the quality of surgical education. But filming in open surgery is not so easy. The cameraperson has to be well trained to catch good takes in the depth of a pit between the heads, shoulders, and fingers of the surgeon and assistants. Most of the time, those constraints disturb the op- ator's manoeuvres, altering their pedagogical value. With the introduction of laparoscopic surgery (LS), using a video camera providing images in real time on a television screen, everything changed.
