Le aritmie costituiscono una branca affascinante e complessa della cardiologia e un campo di studi estremamente interessante.A volte sono la conseguenza di malattie cardiache, in altri casi si riconoscono cause genetiche. Aritmie apparentemente simili spesso hanno un diverso significato predittivo a seconda delle condizioni del singolo paziente, per cui l'approccio non può prescindere dal contesto clinico in cui l'aritmia si presenta.Talvolta, i disturbi del ritmo cardiaco decorrono in modo silente; in altri casi compromettono la qualità della vita. Non sempre comportano un pericolo immediato, ma, non di rado. conducono a situazioni di emergenza o sono causa di morte improvvisa. Questo aspetto impone spesso scelte terapeutiche immediate e corrette.Nel corso degli ultimi anni le conoscenze sui disturbi del ritmo cardiaco hanno avuto una continua evoluzione in tutti i loro aspetti fondamentali.Attraverso studi sperimentali e clinici, alle conoscenze tradizionali si sono aggiunte nuove acquisizioni concernenti i meccanismi aritmogeni, la diagnosi e il trattamento che attualmente si avvale di farmaci, di terapie elet-triche, della neuromodulazione e dell'ablazione transcatetere del substrato aritmico, risorse impensabili fino a pochi anni fa.Le tecniche di stimolazione permanente hanno registrato continui progressi con l'introduzione dei pacemaker leadless e la ricerca di nuovi siti di sti-molazione. Del pari si è arricchito il bagaglio delle conoscenze sulle malattie dei canali ionici e sulle miocardiopatie aritmogene su base genetica, un campo in continua evoluzione.Le tecniche di registrazione continua dell' elettrocardiogramma hanno aumentato il potenziale diagnostico; il monitoraggio remoto dei dispositivi medicali, collegando l'ospedale con il territorio, costituisce ormai una vera e propria continuità assistenziale.L'elettrocardiografia dinamica ha permesso di evidenziare la grande diffusione delle aritmie non solo nei cardiopatici, ma in ogni tipo di popolazione e in soggetti di ogni età della vita e perfino in individui apparentemente normali e negli sportivi.Ciò ha portato allo sviluppo dell'aritmologia pediatrica e geriatrica, dell'aritmologia dello sport, della cronobiologia delle turbe del ritmo.Sono stati anche meglio definiti gli effetti aritmogeni dei farmaci antiaritmici, vecchi e nuovi, e i meccanismi della sincope e della morte improvvisa. L'elettrofisiologia, da indagine prevalentemente diagnostica, con l'avvento dell'ablazione transcatetere ha acquisito un ruolo fondamentale nella terapia delle aritmie. Infine, l'intelligenza artificiale si sta rivelando un importante alleata del cardiologo per la diagnosi e la gestione delle aritmie cardiache.Questi vari accadimenti hanno acceso nuovi entusiasmi per l'aritmologia e hanno accresciuto la necessità di coinvolgere sempre più, nell'assistenza al paziente aritmico, accanto al medico curante e al cardiologo clinico, l'aritmologo, l'elettrofisiologo, il genetista, il farma-cologo, il bioingegnere.Il momento più esaltante per un team medico è il poter trasferire una così vasta potenzialità di risorse a chi soffre, conservando all'approccio al paziente aritmico una imprescindibile base clinica e un sincero e precipuo interesse nel volerne migliorare la sopravvivenza e la qualità della vita.Sulla base di queste considerazioni ho ritenuto esserci dei motivi validi per dare vita a una nuova edizione di questo manuale.
