«Il primo trapianto di cuore, eseguito da Christiaan Barnard nel 1967, ha aperto una nuova era nel trattamento chirurgico dello scompenso cardiaco terminale. Gli anni in cui sono stati effettuati i primi trapianti cardiaci sono stati caratterizzati da grande entusiasmo sia da parte dei chirurghi che del pubblico. Per mantenere le sue promesse ed aspettative si è dovuto attendere fino all'inizio degli anni ottanta, quando sono divenute disponibili nuove metodiche per controllare il rigetto tissutale. Da allora il trapianto è passato da una procedura sperimentale ad una terapia consolidata non più considerata inconsueta. Ogni anno, circa 2500 trapianti di cuore sono effettuati nei soli Stati Uniti. Tuttavia, il numero di donatori disponibili di cuore è limitato rispetto al numero di pazienti che necessita un trapianto e molti pazienti muoiono in attesa del cuore del donatore. Per questo motivo, i dispositivi di assistenza ventricolare (VAD) sono ampiamente utilizzati per fornire un temporaneo "ponte" per tenere in vita il paziente fino a che un cuore di un donatore non divenga disponibile. Un dispositivo di assistenza può aiutare il paziente a recuperare abbastanza per diventare un candidato migliore al trapianto. Tuttavia, non tutti i pazienti con insufficienza cardiaca avanzata sono candidati per un trapianto.
