Questa è anche una occasione per salutare e ringraziare Prof. Giovanni B. Agus, che ci ha prematuramente lasciati a fine agosto e sarà purtroppo un vuoto incolmabile, soprattutto per chi come me lo ha avuto come Maestro per oltre 40 anni, iniziando dalla Scuola di Specializzazione di Chirurgia Vascolare all’Università di Milano. Come sempre e anche in questo libro, lo scopo è di far giungere le nozioni aggiornate pratiche ed essenziali, oggi si usa dire “agili”, che riguardano le patologie della coagulazione del sangue e dei vasi sanguigni, arteriosi e venosi. Viviamo in un’epoca dove internet ed i motori di ricerca sono i principali strumenti di “lavoro e consultazione”, sia da parte dei Medici che dei Pazienti. Se da un lato i primi sono frequentemente obbligati a fare i “tuttologi” i secondi spesso chiedono o suggeriscono trattamenti e terapie sulla base di “notizie lette su motori di ricerca”, e qui nasce e prolifera confusione, che si è intensificata dopo i due terribili anni della pandemia Covid. Lo scopo di questo libro è quindi di trattare il vasto capitolo delle patologie artero-venose ma soprattutto delle terapie mirate cioè sulle categorie di antitrombotici, anticoagulanti ed antiaggreganti, come affermato dallo stesso Titolo. Può essere assurdo, per non dire incredibile, quante volte si creano e tramandano terapie antiaggreganti con lo scopo di ottenere risultati anticoagulanti, e vice versa. Pure sugli antitrombotici c’è una nebulosa che offusca i veri meccanismi d’azione dei farmaci, a tal punto che questi vengono consigliati inappropriatamente. La confusione poi si complica quando non c’è distinzione tra “farmaci ed integratori”. Il problema forse maggiore risulta nell’uso e quindi prescrizione degli anticoagulanti, siano essi quelli iniettivi eparinici che quelli orali di ultima generazione. I Prodotti sono tutti validi, ma è il loro impiego, la posologia, la durata della terapia e la richiesta di esami ematochimici e strumentali inutili, a rendere complicato l’obiettivo primario: la guarigione del Pazienti con la spesa corretta e praticità reciproca per Medico e Paziente. Oggi per un paziente ottenere una terapia adeguata e prolungata, noto anche come “piano terapeutico” può significare un calvario di ore di attesa dopo settimane o mesi per ottenere un appuntamento. Le conseguenze spesso finiscono con sovraccarichi per i Pronto Soccorso. Questo libro racchiude l’esperienza di Professionisti con la conoscenza della Clinica, la Semeiotica e la Terapia sia essa medica, chirurgica o strumentale, con gli adeguati mezzi per raggiungere gli obiettivi e soprattutto evitando perdite di tempo e sprechi economici. Ricordiamoci sempre che non esiste Patologia, qualunque essa sia, che non abbia o potrebbe avere implicazioni o complicanze vascolari e quindi si deve sempre essere pronti ad affrontarle.
