Avevo deciso di fare un viaggio autofinanziato in Inghilterra, frequentavo il Queen Square Hospital al mattino e nel pomeriggio andavo alla British Library alla ricerca di libri di fisioterapia con l’obiettivo di espandere le mie conoscenze. Quando mi trovai in mano il libro Differential Diagnosis in Physical Therapy (terza edizione) rimasi folgorato. Era la soluzione dell’enigma che stavo vivendo come professionista. Il libro conteneva le conoscenze che mi mancavano per diventare un professionista veramente in grado di gestire in autonomia i pazienti. Quelle conoscenze mi avrebbero reso completamente sicuro nell’esercitare le competenze e nell’adempiere alle responsabilità della mia professione. Appena rientrato in Italia, curai quella che sarebbe stata la prima edizione italiana del libro. Negli ultimi vent’anni, ho continuato ad approfondire il concetto della diagnosi differenziale in fisioterapia, la mia passione è immutata e sono ancora qui a occuparmi di questa materia.La diagnosi differenziale per i fisioterapisti è essenziale indipendentemente dal contesto di esercizio professionale.In rapporto all’invecchiamento della popolazione, i pazienti presi in carico dai fisioterapisti sono sempre più complessi e portatori di molteplici comorbidità. Questi pazienti presentano un maggior rischio di manifestare nuove condizioni, non presenti al momento dell’inquadramento medico iniziale che ha preceduto la presa in carico fisioterapica. Sia nel contesto ospedaliero, dove il paziente è gestito in contemporanea da più professionisti, sia nel proprio studio privato, il fisioterapista potrebbe essere il primo a notare l’esordio di una nuova sintomatologia che richiede l’attenzione medica. La valutazione delle condizioni generali del paziente (un approccio per il quale sono necessarie le competenze della diagnosi differenziale per fisioterapisti) è un processo in continuo nell’iter di cura del paziente.Da sempre, tutti i fisioterapisti fanno precedere al trattamento una valutazione iniziale del paziente che ne stabilisce l’idoneità alla loro presa in carico.In alcuni contesti ospedalieri la presa in carico del fisioterapista è regolata da una richiesta medica specialistica che lascia al fisioterapista una completa autonomia nella valutazione del paziente e nella definizione del programma riabilitativo. Nello svolgere queste competenze esperte, i fisioterapisti raccolgono un’anamnesi, interpretano dati per bioimmagine, esami di laboratorio, ed emogasanalisi per stabilire un programma di fisioterapia sicuro ed efficace.Nel resto degli ospedali italiani, l’attribuzione delle funzioni e delle responsabilità ai fisioterapisti è molto variabile. Si passa da situazioni dove qualsiasi attività deve essere indispensabilmente preceduta da una valutazione fisiatrica ad altre dove possono essere presi in carico direttamente dai fisioterapisti solo i casi a bassa complessità.Si auspica di assistere con il tempo a un completo accoglimento delle reali competenze specialistiche e/o avanzate dei fisioterapisti all’interno della pianificazione del lavoro con l’attribuzione di funzioni specifiche.Tutti i fisioterapisti dovrebbero apprendere la diagnosi differenziale in fisioterapia per una pratica più sicura e per poter collaborare al massimo con il medico nell’interesse più speciale, quello del paziente.È in atto una rivoluzione che porterà con sé un desiderio di aggiornamento e di acquisizione di maggiori competenze per le professioni sanitarie. In questa direzione sono sicuro uno dei primi doveri per tutti i fisioterapisti sarà la capacità di identificare le bandiere rosse nell’anamnesi e nei reperti dell’esame dei propri utenti per riuscire a realizzare programmi di intervento che siano sicuri ed efficaci.Mai come in questo ultimo periodo mi sento di invitare i colleghi a “crederci ancora”.Dott.
