Prefazione La zoppia nel cane è una problematica di comune riscontro nella pratica clinica. Mentre per la medicina equestre esistono innumerevoli studi in letteratura sull’argomento, poche risorse sonodisponibili nell’ambito della zoppia in medicina canina; questo probabilmente è dovuto ad una maggiore frequenza ed importanza della zoppia nella pratica quotidiana della medicina equestre. Tuttavia, con il recente sviluppo delle pratiche sportive canine e grazie alla sempre maggiore attenzione da parte dei proprietari alla felicità e vivacità dei loro cani, il campo dell’ortopedia canina sta cambiando, rendendo sempre più importante la prevenzione e la diagnosi accurata delle malattie ortopediche. Lo scopo di questo libro è quindi di affrontare le lacune della letteratura, fornendo una singola risorsa di informazioni clinicamente rilevanti per i professionisti veterinari che riscontrano problematiche relative alla zoppia nel cane.All’interno della comunità degli ortopedici la zoppia è definita generalmente come un’alterazione della normale locomozione che può essere dovuta a dolore (ad esempio in caso di artriteo frattura), disfunzione meccanica (ad esempio in seguito a contratture muscolari), o condizioni neurologiche (ad esempio in caso di compressione del midollo spinale per una patologia del disco intervertebrale; Renberg 2001; Baxter e Stashak 2011). Tuttavia, all’interno della comunità dei neurologi la zoppia è più specificatamente definita come un dolore da compressione delle radici nervose e delle meningi (Capitolo 4; Dewey et al. 2016). Questa “zoppia neurologica”e la monoparesi (cioè malattia del motoneurone inferiore di un singolo arto) sono le agioni più comuni di confusione tra disturbi ortopedici e neurologici. Ai fini di questo libro il termine zoppia si applica al suo significato più ampio, se non diversamente specificato (come nei Capitoli 4, 16 e 21).Questo libro è suddiviso in due sezioni: la Sezione 1 si concentra sulla valutazione della zoppia, descrivendo i diversi tipi di esami (ad esempio l’esame ortopedico e neurologico, ecc.) che aiutano nella localizzazione di un problema che causa un’anomalia nell’andatura (Parte I) e le raccomandazioni relative alle procedure diagnostiche che aiutano a stabilire una diagnosi definitiva (Parte II). La Sezione 2 descrive le cause più comuni di zoppia, organizzate in base alle regioni anatomiche della porzione distale degli arti (Parte III), dell’arto toracico (Parte IV) e dell’arto pelvico (Parte V), comprese le articolazioni principali e le strutture circostanti. Per facilità di identificazione questi capitoli sono contrassegnati con delle etichette colorate. L’organizzazione del libro è stata scelta per fornire una guida che imita l’ipotetico scenario clinico in cui l’esaminatore prima identifica un’anomalia (es: dolore o gonfiore) in un’area specifica e successivamente sviluppa una lista di diagnosi differenziali e un piano diagnostico basato su dati raccolti, così ogni capitolo regionale si concentra sulle patologie muscolo-scheletriche della specifica area. Le altre due cause di zoppia, vale a dire oncologica e neurologica, sono descritte nei singoli Capitoli 16, 17, e 21, 22 per l’arto toracico e pelvico, rispettivamente.L’enfasi di questo testo è posta sul fornire le risorse necessarie per stabilire una diagnosi per i cani che presentano zoppia, con una particolare attenzione all’esame fisico e ai riscontri radiografici. Anche se le condizioni neurologiche generanti zoppia sono incluse in questo libro, lo scopo dello stesso è limitato a descrivere le condizioni di tale natura che possono essere confuse con una zoppia ortopedica. Per questo motivo, le condizioni che causano evidenti anomalie neurologiche su più arti sono discusse solo nella maniera in cui si applicano come una possibile diagnosi differenziale di zoppia.Il lettore è invitato a consultare altre risorse per ulteriori dettagli sulle patologie neurologiche. Allo stesso modo, lo scopo di questo libro non è quello di fornire una descrizione anatomica dettagliata di ogni regione, ma piuttosto di limitare le informazioni a quelle più clinicamente rilevanti. I manuali anatomici veterinari saranno una risorsa utile al lettore che desidera descrizionianatomiche più dettagliate. Inoltre, dal momento che questo libro si concentra sulla diagnosidi zoppia, le opzioni di trattamento sono solo brevemente menzionate. Il lettore dovrebbe consultare altri testi per informazioni più dettagliate sulla gestione di ogni singola patologia. Questo “fondamentalismo” scientifico fa del libro una guida sicura, forte e chiara, che può condurre il medico attraverso le sabbie infide di un argomento vasto e non sempre ben definito come le zoppie. In aggiunta, il lettore attento potrà apprezzare in ogni punto del testo la convinzione dell’autore che un buon procedimento diagnostico nasce da una conoscenza interdisciplinare molto approfondita di tutti gli argomenti che possono riguardare un dato campo di analisi, senza limitarsi alla sola ortopedia, come la neurologia, la fisioterapia o la diagnostica per immagini, per citare alcuni esempi di discipline che analizzano le zoppie e, più in generale, le patologie dell’andatura. Inoltre ogni argomento è trattato fornendo numerosi e precisi ragguagli di letteratura, consentendo al lettore di poter ampliare facilmente le sue conoscenze.
