A questo si aggiunge che, seppure la pandemia COVID-19 ha aumentato l’attenzione e la sensibilità sul problema, e notevoli miglioramenti sono stati fatti e si stanno facendo in ambito sia diagnostico che terapeutico, l’approccio mirato alla gestione delle infezioni batteriche e fungine continua a rappresentare una sfida costante per il clinico. E solo attraverso un percorso multidisciplinare le professionalità combinate di intensivista, infettivologo, microbiologo, farmacista possono integrarsi e completarsi. Il manuale che abbiamo realizzato deriva proprio dalla collaborazione di alcuni dei maggiori esperti sul tema, in vari ambiti e discipline, ed affronta, nella sua prima parte, la tematica delle infezioni di difficile trattamento in Terapia Intensiva a partire dagli aspetti più generali, quali i fattori di rischio, le misure di infection-control e di isolamento in ambito intensivo, le tecniche di approccio diagnostico e le basi per un approccio ragionato all’antibiogramma. La seconda parte, basata su un approccio sindromico, affronta gli aspetti più specifici della gestione clinica del paziente settico ed immunodepresso, delle infezioni respiratorie ed intra-addominali di difficile trattamento. Chiudono il manuale una serie di capitoli in cui vengono trattate le caratteristiche fisiopatologiche, cliniche e diagnostiche delle infezioni causate da Enterobacterales, Acinetobacter baumannii, Pseudomonas aeruginosa, Staphylococcus aureus meticillino-resistente, Enterococcus spp e fungine di crescente interesse e rilevanza clinica. Tra le infezioni nosocomiali rilevate nelle Unità di Terapia Intensiva, le più frequenti e temute sono le polmoniti associate a ventilazione meccanica (VAP, Ventilator Associateci Pneumoniae), responsabili di elevata mortalità, morbilità, tempi prolungati di svezzamento dal ventilatore ed ampio utilizzo di terapia antibiotica. Si definisce VAP l'infezione del parenchima polmonare in pazienti intubati e ventilati meccanicamente da almeno 48 ore; Essa è infatti un'importante complicanza della ventilazione meccanica associata a esiti avversi e costi di assistenza più elevati. La mortalità attribuibile stimata di VAP è di circa il 10%, con una maggiore incidenza nei pazienti di sesso maschile, con ARDS, BPCO, lesioni cerebrali, supporto ECMO. L'identificazione dei fattori associati allo sviluppo e alla prevenzione della VAP è essenziale per medici e pazienti. La conferma microbiologica dell'infezione è data dall'identificazione del patogeno a un esame microbiologico su campioni raccolti nelle vie aeree distali tramite lavaggio broncoalveolare. Mentre la radiografia del torace ha una scarsa precisione diagnostica, l'ecografia polmonare è utile per la diagnosi precoce dell'infezione al letto del paziente e per monitorare l'efficacia della terapia antibiotica. Il trattamento deve essere selezionato tenendo conto del livello di rischio di multi-resistenze e del rischio di mortalità. Gli antibiotici devono essere descalati non appena è disponibile l'antibiogramma o sospesi se le colture risultano negative. Nonostante i progressi nella terapia antibiotica, nell'imaging e la diffusione di strategie di prevenzione, la VAP resta ad oggi un'importante causa di mortalità, prolungamento della degenza in ospedale e significativo incremento dei costi dell'assistenza.
