In 2002, Lippincott published the Manual of Breast Diseases, edited by Professor Ismail Jatoi. The current book, Management of Breast Diseases, is an adaptation of that manual, with Professor Manfred Kaufmann of the Goethe-University of Frankfurt serving as co-editor. Most of the chapters from the original manual have been either extensively revised or discarded, and several new chapters added. This text contains more material than the original manual, but it is still intended as a basic guide for the wide spectrum of clinicians (surgeons, gynecologists, oncologists, radiation onco- gists, internists, general practitioners) who treat breast diseases, both benign and malignant. To compile this text, we assembled experts from throughout the world. Thus, this text provides not only a broad overview of breast diseases, but also highlights diff- ent perspectives from different parts of the world. Yet, it is worth noting that the management of breast cancer is now largely predicated on evidence-based medicine. Several large, randomized prospective trials have demonstrated the ef? cacy of breast cancer screening and chemoprevention. Other large trials have addressed the impact of systemic therapy, radiotherapy, and variations in local therapy on breast cancer mortality. Many of these landmark trials are discussed in this text, and they clearly have had a bene? cial effect. Indeed, since about 1990, breast cancer mortality rates have declined substantially in most industrialized countries, and this trend is expected to continue in the years ahead.

"Qual è il significato reale di chirurgia oncoplastica nei tumori della mammella? In effetti, è una parola un po' inflazionata usata molto a volte con appropriatezza altre volte magari senza raggiungere il vero obiettivo e significato di quello che questa frase racchiude. Sicuramente partirei da un consensus che è stato tenuto a Firenze nel 1998 dove una serie di esperti italiani sia di chirurghi generali e senologi sia di chirurghi plastici si è riunita e ha cercato di stabilire quali fossero le condizioni essenziali per la chirurgia ricostruttiva della mammella. Bene la frase conclusiva che ha raccolto questo consensus è riassumibile in poche parole, cioè che la chirurgia ricostruttiva della mammella è, di fatto, una parte integrale del trattamento chirurgico del tumore mammario e questo direi che è importante e ci spiega in passo successivo all'oncoplastica".

Se dovessi qualificare questo manuale con un unico termine, sceglierei senz’altro l’esaustività. Si tratta di un manuale tecnico di alta qualità che affronta con attenzione e scientificità ogni aspetto e fase legati al tumore al seno. Partendo dalla concezione che il tumore della mammella è una malattia eterogenea che si presenta sotto diverse forme e in donne che hanno caratteristiche biologiche, sociali ed umane differenti viene tracciata l’anatomia della mammella e con l’epidemiologia vengono definiti i fattori di rischio che possono concorrere all’insorgenza del tumore al seno, vengono identificati i vari tipi di tumore ed i traguardi della ricerca scientifica in campo diagnostico e terapeutico. Una visione così ampia è necessaria per sottolineare l’approccio oggi multidisciplinare al tumore della mammella: l’analisi dell’assetto istopatologico, del profilo genetico e dei determinanti endocrini, sono infatti parte integrante delle decisioni e strategie cliniche e terapeutiche per la scelta del trattamento pensato su misura del paziente e per il migliore controllo della malattia.L’impegno e sforzo degli Autori è chiaro: vi è la volontà di evidenziare come la scienza si sia oggi posta dalla parte della difesa dell’integrità fisica e psicologica della donna abolendo il dogma del ‘massimo trattamento tollerabile’ per ricercare il ‘minimo trattamento efficace’. Ma una corretta comunicazione scientifica in campo umano è altrettanto essenziale e necessaria per promuovere atteggiamenti corretti e una giusta sensibilizzazione nella popolazione. Infatti una comunicazione anche solo imperfetta potrebbe pregiudicare i vantaggi che la ricerca oggi offre. Ed il manuale diventa, non più solo scienza, ma coscienza, poiché sottolinea il ruolo fondamentale del medico nella relazione con il paziente, spesso fatta di empatia e condivisione, che non può prescindere da un lato dal rispetto di valori etici e morali nella considerazione della malattia in situazioni delicate e difficili, come la scoperta di un tumore in stato di gravidanza o in casi particolari, come la diagnosi di un tumore della mammella maschile e dall’altro dalla volontà della persona.Mi sento di potere affermare che questo lavoro sia nato con l’obiettivo di informare che oggi il tumore al seno è un male sempre più curabile: la tempestività delle diagnosi e i progressi sul fronte della chirurgia e dei trattamenti hanno portato a risultati incoraggianti che possono diventare ulteriormente promettenti se affiancati dalla responsabilizzazione della persona nella difesa della propria salute.Umberto Veronesi
