Laureato a Milano, rientrato in Italia nel 1964, dopo un tirocinio al Memorial Sloang Kettering Cancer Center di New York, ha sempre lavorato presso l'Istituto nazionale dei Tumori di Milano, dove nel 1976 è stato nominato direttore della Divisione di Oncologia medica e, successivamente, direttore del Dipartimento di Medicina oncologica. Le sue ricerche di maggior rilievo comprendono le prime valutazioni cliniche sull'efficacia dell'adriamicina, vari studi sulla chemioterapia adiuvante e primaria del carcinoma mammario, sulla malattia di Hodgkin, per il cui trattamento ha messo a punto una nuova combinazione di farmaci, nota come ABVD. Nel 1995, un ictus ha interrotto bruscamente la sua carriera professionale. Nel 2013, ha lasciato definitivamente l'Istituto dei Tumori di Milano, per dedicarsi alle attività della "Fondazione Michelangelo", organizzazione non lucrativa di utilità sociale per l'avanzamento dello studio e della cura dei tumori, che egli stesso ha fondato nel 1999. L'American Society of Clinical Oncology ha istituito il "Gianni Bonadonna Award and Lecture" come riconoscimento dell'importante attività clinico-scientifica svolta dal grande medico. Bonadonna ha ricevuto varie onorificenze nazionali e internazionali e nel 2004 è stato insignito della laurea honoris causa in Medicina dall'Università di Torino.
