Progressi tecnici hanno portato allo sviluppo della RM-colangiopancreatografia, alla tomografia computerizzata spirale, alla colonografia virtuale, al power color-Doppler e all’ecografia armonica, alla risonanza magnetica addominale in apnea e alla fluoroscopia digitale. Ora si stanno usando più ampiamente mezzi di contrasto per risonanza magnetica epato-specifici e mezzi di contrasto per ecografia. Sono stati compiuti anche notevoli progressi nella nostra comprensione della fisiopatologia di un certo numero di malattie, dal ruolo della genetica nell’eziologia del cancro del colon al riconoscimento dell’importanza di Helicobacter pylori nello sviluppo della patologia gastroduodenale benigna e maligna. Per mantenersi al passo con questi progressi tecnologici e scientifici, ogni capitolo della seconda edizione è stato aggiornato e rivisto in modo significativo, ne sono stati aggiunti o eliminati diversi e quasi un terzo dei capitoli ha nuovi autori per dare ai loro argomenti un’impronta più moderna. In tutta questa nuova edizione abbiamo avuto grande cura di mantenere gli obiettivi fondamentali della prima: fornire una trattazione completa e aggiornata dello stato di conoscenza in radiologia gastrointestinale in modo pratico e utilizzabile. Come nella prima edizione, il nostro principio organizzativo di base è stato quello di integrare informazioni rapidamente mutevoli, buon senso e giudizio assennato in un approccio ordinato e pratico alla diagnosi e terapia radiologica. A tal fine, il testo contiene sezioni su principi radiologici generali per valutare i visceri cavi e gli organi solidi, nonché per eseguire e applicare tecniche specifiche di diagnostica per immagini e di radiologia interventistica. Altre sezioni presentano gli aspetti clinici, radiologici e anatomopatologici delle malattie dei vari organi gastrointestinali. Questi capitoli mirano ad illustrare e a integrare lo spettro di anomalie osservate in tutte le modalità diagnostiche disponibili al radiologo: radiografie dirette, esami baritati, colangiografia, tomografia computerizzata, ecografia, risonanza magnetica nucleare, scintigrafia e angiografia. Per rendere il trattato più gradito all’utente, gli autori e i curatori dell’opera hanno lavorato diligentemente per eliminare sovrabbondanze e produrre un testo più pratico.

The potential capacity of the biphosphonates to intervene in osseous remodeling was recognized more than 30 years ago. The biphosphonates inhibit osteoclastic activity. In spite of advances in early detection, surgey, chemo-and radiotherapy, metastatic bone disease is still a major and in many cases intractable clinical problem. The biphosphonates are now the most effective agents in the management of bone metastases - as shown by trials in patients with breast and prostate cancer, and in multiple myeloma. Bone pain, new ostelytic events and pathologic fractures are decreased and complications associated with these events are avoided. Existing osteolytic lesions may even be reversed. New fields of indications of biphosphonates are shown to be in the adjuvant therapy of breast cancer and in stage 1 multiple myeloma to prevent skeletal destruction and also to inhibit tumor growth. Over the past few years the biphosphonates have proved their value also in many other conditions such as Paget's disease of bone, osseous disorders with an osteoclastic component leading to osteopenia/osteoporosis, in congenial, endocrine and metabolic disorders.
