The second edition of this book describes the clinical indications of NIV in patients hospitalized with high-risk infections as well as in the prehospital management of mass casualty incidents, including chemical or biological disasters and pandemics. In recent decades, we have learned the impact that different pandemics and mass casualty disasters can outcome in terms of health resource use, health costs and human lives. The development of respiratory failure in these patients, either infectious or non-infectious causes, has led to develop employment plans related both to invasive or noninvasive mechanical ventilation during acute respiratory failure. In this book authors evaluate a rational basis for indications, specific noninvasive mechanical ventilation indications in hospitalized patients (tuberculosis, bacterial, virus, etc.) and prehospital applications (mass casualty: chemical, biological disaster), equipment (ventilators, interfaces) and plan organization for health systems: how and when apply NIV. But prejudice does not begin or end with disease, and skin color, of course, has been used as an illegitimate but powerful weapon in the service of the most disgraceful ideologies. Meanwhile, our fixation with body image turns out to be no contemporary fad, nor is there anything new in the erotic impulses of humankind. Durante questo periodo si sono svolte innumerevoli vicende personali, professionali e accademiche, ma non ho mai dimenticato la promessa fatta al Prof. Ettore Masenti, che aveva presentato la prima edizione, di aggiornare costantemente il testo. In questa nuova edizione è ampliata sia la storia di Torino e del Piemonte sia quella della Chirurgia piemontese che viene messa a confronto con quella internazionale, in modo che i chirurghi piemontesi possano essere immersi nel loro tempo, insieme con i chirurghi di altre nazioni e agli eventi storici e quotidiani che li hanno accompagnati. Anche questa edizione, come le precedenti, è dedicata al mio Maestro Angelo Emilio Paletto, che mi ha lasciato “il segno e il metodo” come Antonio Carle scrisse del suo Maestro Theodor Billroth. Auguro buona lettura a tutti, ma soprattutto ai chirurghi più giovani, ricordando che la conoscenza delle proprie radici è il motore che spinge verso il futuro.
