Gli argomenti trattatati sono completi e spaziano in tutti gli ambiti della Chirurgia, dall’upper GI al lower GI, dalla chirurgia toracica alla proctologia, dalla chirurgia endocrina alla chirurgia oncologica. La sutura, che consiste nel riunire i tessuti divisi da una ferita o da un’incisione, è una delle pratiche più antiche e fondamentali della chirurgia. Essa ha lo scopo di favorire la guarigione, prevenire le infezioni, ridurre le perdite di sangue e migliorare l’aspetto estetico della cicatrice.Tuttavia, la sutura non è una pratica semplice e banale, ma richiede conoscenze teoriche e abilità pratiche da parte del medico che la esegue.La cicatrice, che inevitabilmente residua dopo qualsiasi intervento chirurgico, è la sola cosa visibile che accompagna il paziente per tutta la vita e, nella maggior parte dei casi, è direttamente legata alla qualità della sutura.L’esito cicatriziale, infatti, dipende in gran parte dalla cura destinata a questa importantissima fase di ogni operazione. Quando il medico applica i punti di sutura durante e, soprattutto, alla fine dell’intervento, sta letteralmente “firmando” il suo lavoro. Deve quindi dedicare ogni sforzo ad eseguire questa parte dell’intervento in maniera corretta e con la tecnica più appropriata. Anche se la procedura è stata lunga e complessa, non ci si deve dimenticare che una cicatrice di scarsa qualità costituirà agli occhi del paziente una sorta di fallimento, perlomeno parziale, dell’intera operazione. Non ci sono giustificazioni valide per una sutura eseguita in modo inadeguato o con una tecnica inadatta. Tanto più che il tempo di esecuzione di una sutura impeccabile e di un rattoppo approssimativo è quasi sempre il medesimo. Quindi, è fondamentale per ogni medico, ancor più se la sua attività è orientata alla chirurgia plastica, conoscere le moltissime tecniche di sutura e applicare in ogni occasione quella più indicata con consapevolezza e precisione. Questa monografia rappresenta una fonte preziosa di informazioni fondamentali per scegliere e applicare la tecnica di sutura più indicata. La Chirurgia Vascolare nasce in Italia nei primi anni ’60, segue un percorso parallelo alla cardiochirurgia, di cui si sente figlia minore, ed evolve lentamente sino ai primi anni ’90 prevalentemente all’interno dei reparti di Chirurgia Generale. È da qui, tuttavia, che si assiste ad una progressiva crescita degli specialisti vascolari, i quali affermano la loro indipendenza grazie al numero sempre maggiore di interventi per la correzione di patologie che spaziano dal distretto aortico a quello carotideo, passando per la rivascolarizzazione degli arti ed a tutta la flebologia. È negli ultimi 20-30 anni che si assiste ad un’esplosione di trattamenti grazie all’avvento della chirurgia endovascolare, la quale ha permesso, attraverso materiali e tecnologie sempre più raffinate, di ampliare le soluzioni tecniche a fronte di minore invasività, garantendo comunque risultati stupefacenti nel breve e lungo termine. The proposal of writing a book dedicated exclusively to the pathology and surgery of the nasal septum fulfills a training gap, which was greatly waited for so far. Pathology and surgery involving the nasal septum have been the major topic of study since 1905 when Killian (Killian G. The submucous window resection of the nasal septum. Ann ORL 1905; 14: 363) described the first technique for correcting deviations. The purpose of this textbook is therefore not to propose some novel practice in the field, but to gather in a single volume the overall approach to septal pathology and surgery, which grew up in a translational and multidisciplinary fashion over this latest century. The impact with the pathologies of the nasal septum is almost immediate as just as the Medical Specialization School in Otolaryngology has started, but also for those practitioners who are introducing to approach Maxillofacial Surgery or Plastic and Reconstructive Surgery. Yet, despite the enormous spread of these pathologies, the high number of professionals who dedicate themselves to them, the very long experience of many schools and also the considerable contribution that Distinguished Italian Masters have given to the topic, we have not yet managed to standardize indications, surgical techniques, management of failures and complications, so far.
