editore: Minerva Medica
pagine: 168
Cardiovascular imaging has faced a momentous development in the last few decades thanks to refinements in imaging technologies, multimodality integration, and focus on different clinical applications ranging from prevention to diagnosis, prognosis, decision making, and follow-up. These developments equally apply to standard non-invasive imaging modalities, including echocardiography, duplex ultrasound, cardiac magnetic resonance, computed tomography, and nuclear cardiology. Most recently, breakthroughs in data science, machine learning and artificial intelligence have been applied to pre and post-processing of imaging data. This synergy between refined cardiovascular imaging methods and novel computing approaches holds the promise of enhanced diagnostic and prognostic accuracy, as well as improved clinical decision making with an ever increasing focus on individualized cardiovascular care.

Percutaneous management of coronary artery disease beyond stents. The future is now
Bernardo Cortese
editore: Minerva Medica
pagine: 374
Interventional cardiology achieved huge improvements in the past decades, and stent technology reached high standards in terms of acute and long-term performance. But these devices are not the panacea of coronary artery disease treatment. Any planned intervention in your patient should be justified by a real need-this is clear cut for any physician! But we want to stress that any prosthesis (foreign body) you would like to implant should be justified by a clear benefit on the very long term. When we propose it, try to predict how many device-related events are expected after 10 years, then ask yourself: is it worth it? In fact, several long-term studies have shown how after the first 18 months, any vascular prosthesis will yield a low but persisting yearly event rate. Keep this in mind. In the first part of the book we will go through an analysis of the pathophysiological milieu of coronary artery disease and an up-to-date analysis of the technological and technical improvements of all the revascularization options that we have for our patients. Then we will go through a deep analysis of all the imaging techniques we have to better visualize and understand what is there, in order to correctly plan what to do. In the following section we will me more practical: what to do if we have large calcifications and how to manage a correct lesion preparation, with all the tools that we can count on our shelves? We do not want to keep the things half way, so now we have to decide which should be the last treatment: a stent, a drug-coated ballon, a vascular scaffold, or a blended therapy?

Since its clinical introduction almost 20 years ago, cardiac CT has been embraced as an indispensable noninvasive cardiac imaging modality and an important first-line test for coronary artery disease. Beyond coronary artery disease, the potential of cardiac CT has become evident for diagnosis and guidance of treatment in a variety of other cardiac pathologies, including valvular disease, atrial fibrillation and other arrhythmias, endocarditis, cardiac masses, cardiomyopathies, and others. The European Association of Cardiovascular Imaging (EACVI) has declared as one of their foremost priorities to facilitate education and training in cardiovascular CT through teaching courses, congresses, and a structured certification program. This handbook represents an important step towards dissemination of skills and knowledge in cardiovascular CT. It is a concise and practical companion, to benefit students, trainees or advanced users; cardiologists, radiologists, cardiac surgeons or technicians, in their everyday practice.Four broad sections cover technical aspects and physical background, coronary indications (e.g. coronary CT angiography, atherosclerosis imaging, stents & bypasses, functional CT imaging, etc.), non-coronary indications (CT for valve disease, infective endocarditis, CT of the left atrium, congenital heart disease, cardiac masses, extracardiac findings, etc.), and finally training and competence in cardiac CT. The handbook features short chapters, enriched with illustrations, tables and condensed summaries, facilitating rapid and intuitive access.
