Over the past two decades it has been increasingly recognized that whole-body ultrasound is an invaluable tool in the critically ill. Knowledge about the etiology and diagnosis as well as treatment concepts of neu- oncologic diseases is rapidly growing. This turnover of knowledge makes it dif? cult for the physician engaged in the treatment to keep up to date with current therapies. This book sets out to close the gap and pursues several innovative concepts. As a comprehensive text on neuro-oncology, its chapters are interconnected, but at the same time some chapters or subdivisions are so thoroughly assembled that the whole volume gives the impression of several books combined into one. Neuropathology is treated in an extensive and clearly structured section. The int- ested reader ? nds for each tumor entity the latest well-referenced consensus rega- ing histologic and molecular pathology. Through this "book-in-the-book" concept, information on neuropathology is readily at hand in a concise form and without ov- loading the single chapters. Pediatric neuro-oncology differs in many entities from tumors in adult patients; also, certain tumors of the CNS are typically or mainly found only in the child. Therefore, pediatric neuro-oncology was granted its own, book-like section.
Tumor entities that are treated differently in children and adults are included both in the pediatric neuro-oncology section and in the general section. Entities that typically occur only in the child and adolescent are found in the pediatric section in order to avoid redundancies.

In 2002, Lippincott published the Manual of Breast Diseases, edited by Professor Ismail Jatoi. The current book, Management of Breast Diseases, is an adaptation of that manual, with Professor Manfred Kaufmann of the Goethe-University of Frankfurt serving as co-editor. Most of the chapters from the original manual have been either extensively revised or discarded, and several new chapters added. This text contains more material than the original manual, but it is still intended as a basic guide for the wide spectrum of clinicians (surgeons, gynecologists, oncologists, radiation onco- gists, internists, general practitioners) who treat breast diseases, both benign and malignant. To compile this text, we assembled experts from throughout the world. Thus, this text provides not only a broad overview of breast diseases, but also highlights diff- ent perspectives from different parts of the world. Yet, it is worth noting that the management of breast cancer is now largely predicated on evidence-based medicine. Several large, randomized prospective trials have demonstrated the ef? cacy of breast cancer screening and chemoprevention.
Other large trials have addressed the impact of systemic therapy, radiotherapy, and variations in local therapy on breast cancer mortality. Many of these landmark trials are discussed in this text, and they clearly have had a bene? cial effect. Indeed, since about 1990, breast cancer mortality rates have declined substantially in most industrialized countries, and this trend is expected to continue in the years ahead.

Musculosketal infections are potentially devastating for the patient, his family and the health care services, as they may lead to functional impairment, long lasting disability, or even permanent handicap. This problem may also have socioeconomic implications. Infection commonly results in prolonged use of antibiotics, multiple operations, complications, extended rehabilitation and long lasting follow-up visits with additional costs for the health care provider. The 2nd International Arthroplasty Symposium "The Infected Implant" brought together international experts and will concentrate on infections: development, microbiology, diagnostics, statistics and therapy of the infected implant. Concerning microbiological diagnostics, a major challenge is the differentiation between septic and aseptic prosthesis loosening. Biofilm formation at low grade infections will therefore be one of our main topics at the event in 2008. Furthermore, the issue of antibiotic resistances, which is always a hot topic,will be discussed, based on new clinical and epidemiological data.These days, a number of options for the therapy of the infected implant are available.
Lectures about the rational use of antibiotic medication and theapplication of antibiotic containing spacers will represent key topics of this meeting. The question whether revision surgery should be cemented or not will be another subject of the panel discussion, where clinicians of different countries worldwide will present their revision cases. Basic research discusses the implication of pancreatic stress protein in acute pancreatitis and pancreatic cancer and their possible role as therapeutic targets. Also, very original results show the unexpected role of lipids as mediators during acute pancreatitis. Gene screening strategies allow the detection of the genes responsible for gemcitabine resistance of pancreatic cancer cells. They lead to the selection of several target genes, in order to suppress the resistance of cells to gemcitabine treatment. The mechanism by which tetrahydrocannabinol is anti-tumoral in pancreatic cancer cells is presented and the use of THC as a promising new therapeutic agent is discussed. Genetic data are shown concerning hundreds of families with hereditary chronic pancreatitis and their possible role in the pathogenesis of the disease. Another very original study addresses the prevention and treatment of pancreatic diseases with diet. In clinical research, convincing data about the use of endoscopic sphicterotomy in the management of acute bilary pancreatitis is presented, based on the experience of a Center highly specialized in pancreatic diseases.

Sepsis is a very important public health problem. It is widely acknowledged that the severe inflammatory response syndrome/severe sepsis paradigm fails to stratify sepsis patients adequately. This book reviews in detail how sepsis should be managed on the basis of a novel approach to staging. The PIRO (predisposing factors, infection, response, organ dysfunction) model was first proposed at the start of the decade on a theoretical basis and has now been translated into a practical approach for use at the bedside. It is loosely based on the TNM staging system for cancers, with points being allocated to each of the four aforementioned key characteristics of the septic process. The proposed PIRO framework facilitates evaluation of factors that are important in the pathogenesis of severe sepsis and in the development of treatment strategies.
