Cardiac magnetic resonance is nowadays considered the gold standard noninvasive imaging technique in different cardiac disease. In particular, cardiac MR is a fundamental diagnostic and prognostic tool for the diagnosis and characterization of heart disease that increase the risk of sudden cardiac death, due to the ability to identify even small abnormalities of myocardial tissue, such as scar of fat infiltration. Accordingly, the recent (2023) European Society of Cardiology GuideLines on Cardiomyopathies recognized the performance of cardiac MR in all suspected or known cardiomyopathies with Level 1 of Recommendation. In the growing field of Sport Cardiology, the Italian Cardiological Guidelines (COCIS) for Competitive Sport Eligibility in athletes with heart disease strongly recommends the use of Cardiac MR in all athletes wit significant ventricular arrhythmias or suspected myocardial disease. Finally, cardiac MR remains the standard-of- reference diagnostic tool for assessing myocardial viability in ischemic heart disease.
