Biomarkers of Disease
An Evidence-Based Approach
Andrew Trull
,
Atholl Johnston
e altri
editore: Cambridge
pagine: 518
This publication takes a critical, evidence-based look at the efficacy of diagnostic tests which are increasingly being used to evaluate organ damage and dysfunction. The use of biomarkers is growing, with a steady stream of products being brought out by the pharmaceutical industry. Some of these assist in diagnosis, others provide a means of monitoring the state of progression of disease and the effectiveness of therapeutic options. However, in many cases the evidence which supports the use of these methods as opposed to traditional biochemical tests has not yet been demonstrated, and it is intended that this volume will help clarify the strengths and weaknesses of using these biomarkers across a wide range of applications and in the various organs of the body. This approach will provide pathologists, clinical biochemists and medical laboratory scientists with an invaluable overview of the diverse applications of biomarkers in medicine.
Optimal Treatment Strategies in End-stage Renal Failure
Claude Jacobs
editore: Oxford University Press
pagine: 320
During the past 4O years there have been major conceptual and technical advances in the domain of dialysis methods and renal/organ transplantation for long-term treatment of patients with End-Stage Renal Failure (ESRF). This now enables better defined, more selective strategies of treatment to be undertaken, according to patient-specific criteria such as age or underlying renal disease also taking into account the lifetime duration of these treatments. For many patients this implies necessary successive changes of mode of therapy depending on their availability, occurrence of medical/technical complications or failure and social-environmental and economic factors. This rationale has inspired the structure of this volume which is divided into four sections: 1/ A descriptive overview of the various modes of renal replacement therapy (RRT): Extracorporeal dialysis/filtration, peritoneal dialysis, kidney and multi-organ transplantation.2/ Most appropriate indications and use of these methods, respective advantages, drawbacks and outcome in children, pregnant women and elderly patients 3/ In diabetic patients and in patients with hereditary/congenital diseases.
4/ The ethical issues generated by this new domain in Medicine by limitations in treatment facilities or medical dilemmas for acceptance, best technical choices, withdrawal or termination of RRT in individual patients. Finally, contributors form Eastern European Countries, Africa and Far Eastern Countries analyse the current status of RRT in their respective geographical area and the ways and means required for a wider implementation of RRT in thus far lesser economically developed countries where the great majority of the populations still have no access to these life-saving therapeutic procedures.
Neurosurgery of Arteriovenous Malformations and Fistulas
A Multimodal Approach
Alexander Muacevic
,
Berndt Wowra
e altri
editore: Springer Verlag GmbH
pagine: 476
Arteriovenous malformations (AVM) and arteriovenous fistulas (AVF) differ from all other pathology affecting the central nervous system by their high-flow arteriovenous shunts. Permanent occlusion of these shunts is the essence and the challenge of therapy. Endovascular therapy and radiosurgery became accepted alternatives or adjuncts to surgery. In many instances the choice of the primary therapeutic modality is not clear and arguments can be found for several options. However, microsurgery, endovascular therapy and radiosurgery differ very much with regard to invasiveness, length of stay at the hospital but also residual risk after therapy. The emerging treatment concepts are the object of this book. The result is a unique structured presentation of AVM and AVF therapy.

Because of the frequency of this problem it has come to be managed by surgeons in general, regardless of the type of hospital or the conditions dealt with in their daily practice. Laparoscopic surgery has demonstrated to have an important role among the different technique described to repair ventral hernia with less recurrent rate, less morbidity and less overall cost than open conventional repair, with all the advange of the laparoscopic approach. As a result the indications for this surgical technique are currently being debated since the advantages are evident and progressive implementation is ensured. Now is the time to analyze the usefulness, results, technical variants, anatomic, physiologic and scientific basis and implications involved in implementation of laparoscopy as the technique of choice.
