PREFAZIONE I continui progressi della tecnologia e dei sistemi sanitari nel mondo sono stati particolarmente impegnativi per la professione infermieristica negli ultimi 10 anni. Tuttavia, credo fermamente che per migliorare la salute non possiamo dimenticare il passato. Compiti fondamentali come l’accertamento infermieristico e la documentazione sono più che mai importanti. L’assistenza al paziente dall’ospedale a casa in qualsiasi struttura ricettiva è essenziale per ridurre i costi e migliorare gli esiti. L’assistenza sanitaria oggi deve rispecchiare il passato, tenendo il paziente al centro dell’interesse, a casa sua, o durante la transizione dall’ospedale a casa, nella sua famiglia o nel sistema di comunità, piuttosto che essere un semplice ingranaggio nella grande macchina del sistema sanitario.  Dopo la pubblicazione della precedente edizione de Il manuale dell’infermiere, ho spostato la mia pratica clinica sull’assistenzainfermieristica a domicilio, con la ferma convinzione che è questo il posto in cui posso sfruttare meglio i miei 40 anni di esperienza per una incidenza ottimale sulla salute e sulla qualità della vita delle persone. Mentre presto assistenza esclusivamente nei servizi a domicilio e nelle strutture residenziali, sono attiva nel coordinamento dell’assistenza al paziente in tutti i setting di cura. Mentre perseguo come obiettivo principale la prevenzione del ricovero ospedaliero, sono attenta ai bisogni di assistenza che diventano più grandi di quanto persino un team sanitario altamente qualificato sia in grado di soddisfare. Quando un paziente è ricoverato, il mio impegno è rivolto a favorire la transizione verso le strutture di riabilitazione e il ritorno a casa. Quando un paziente a casa ha bisogno di un’assistenza nelle attività della vita quotidiana in misura maggiore di quanto ne possa essere fornita in quell’ambiente, io mi adopero per promuovere la transizione verso un setting di assistenza a lungo termine. La sezione sul processo infermieristico fornisce una panoramica pratica dell’assistenza infermieristica passo-passo per quasi qualsiasi scenario del paziente.  Questa edizione è suddivisa in cinque parti al fine di presentare un riferimento completo per tutti i tipi di assistenzainfermieristica. La Parte prima tratta il ruolo dell’infermiere nel sistema sanitario.
