Atherosclerotic cardiovascular disease remains the leading cause of death worldwide, yet our understanding of its biology has evolved more profoundly in the past two decades than in the previous fifty years. Traditional paradigms centred on established clinical scores, ischaemia and flow-limiting stenoses have undoubtedly shaped modern cardiology, but they increasingly revealed intrinsic limitations. Most myocardial infarctions originate from non-obstructive, biologically active plaques that progress silently long before clinical manifestation, and first events frequently occur in individuals classified as low risk by conventional assessment tools. This textbook embraces a modern vision of coronary artery disease, integrating advanced mechanistic knowledge, emerging risk factors, state-of-the-art imaging and contemporary pharmacological strategies to elucidate the underpinnings of modern clinical management of atherosclerosis, from understanding of traditional and non-traditional plaque progression risk factors to defining how these processes can be favourably haltered through targeted interventions. Through contributions from leading experts in cardiovascular medicine, Treating the Atherosclerotic Plaque 2.0 provides a comprehensive and up-to-date synthesis of the groundbreaking evidence that has reshaped our understanding, management and treatment of atherosclerosis. Prefazione“Il compito più difficile nella vitaè quello di cambiare sé stessi”Nelson MandelaNella storia e nella vita comune si parla spesso della professione infermieristica come di una vocazione che alcuni, per fortuna di altri, accettano di seguire e perseguire per bontà d’animo o predisposizione personale. Sarebbe sciocco rifiutare a priori e del tutto questa etichetta che la società ci ha attribuito, perché negheremmo di avere, in quanto infermieri, una predisposizione e un pensiero di amore verso l’uomo. Tuttavia, questa definizione è oggi più che mai inappropriata. Ci piacerebbe dunque che, partendo da questa premessa, non si parli più solo di vocazione, ma di una professione che negli anni ha costruito un corpo di conoscenze, una teoria, un’etica e delle competenze cliniche ed umane attraverso le quali perseguire il suo scopo: tutelare la salute delle persone. Nel nostro immaginario la nuova protagonista diventa la competenza, spesso proferita ed esibita senza piena consapevolezza e cognizione di causa. Così ricercata, così richiesta e desiderata in tutti gli ambiti della nostra vita: da chi costruisce le nostre case a chi governa la nostra quotidianità, a chi educa i nostri figli e, non per ultimi, a chi si prende cura dei nostri cari. Difficile è darne una definizione, ma immaginando un infermiere competente vediamo qualcuno che, preparato, aggiornato, attento, preciso, sensibile e coinvolto assiste con cura coloro che gli sono affidati. Un infermiere che, nello svolgere il proprio lavoro riesce a costruire speranza, tutelando la salute e proteggendo la dignità dell’altro. Questo è quello che desidereremmo per qualcuno a noi caro: un professionista con una buona padronanza delle tante conoscenze tecniche, teoriche e pratiche tipiche della Terapia Intensiva e della Sala Operatoria, ma anche consapevole del proprio ruolo all’interno dell’équipe multidisciplinare, attento alle priorità assistenziali quanto alla relazione con gli assistiti e i loro famigliari. Sempre disposto a mettersi in gioco e aperto al cambiamento culturale imposto da un nuovo paradigma: restituire alle persone la migliore qualità di vita possibile.Nulla di scontato dunque, nulla di semplice, nulla di sostituibile con qualcosa di ripetitivo, tecnico e facilmente riproducibile, ma un lavoro che racchiude appieno la comprensione totale dell’uomo in un momento delicato come quello della sofferenza e della vulnerabilità. Un infermiere competente, che non si costruisce in un giorno, che probabilmente ha davanti a sé ancora molta strada da percorrere, ma del quale siamo tutti coscienti di avere un grande e urgente bisogno.Questo testo vuole essere un passo su questa lunga strada.
