Nel corso della mia attività ho avuto modo di incontrare bimbi non vaccinati, e ho iniziato a rendermi conto che avevo accettato senza riserve e riflessioni il concetto secondo cui i vaccini sono sempre efficaci e sicuri. Ho cominciato a constatare che la frequenza delle malattie pediatriche più comuni erano ridotte nei bimbi non vaccinati rispetto a quelli sottoposti alle vaccinazioni. Ho conosciuto genitori i cui figli si erano ammalati gravemente, o che addirittura erano morti a causa della somministrazione di un vaccino.

La terapia antinfettiva rappresenta uno dei cardini della terapia medica. E' tuttavia paradossale osservare che la ricerca scientifica nel campo dei farmaci antinfettivi ha subito una battuta d'arresto molto pesante negli ultimi dieci anni, come se non vi fosse più nulla da scoprire e l'armamentario terapeutico in questo campo fosse ormai tale da rendere superfluo un ulteriore sforzo nella ricerca. L'autore con questo volume ha inteso creare qualcosa di nuovo perché, da un lato, nuovi farmaci antibiotici e antifungini si sono comunque affiancati alle varie opzioni terapeutiche e, dall'altro, nuove modalità di impiego, nuove linee-guida e nuovi indirizzi applicativi si sono venuti ad aggiungere alle conoscenze finora acquisite. La spiegazione è molto semplice: la controparte della terapia antinfettiva è rappresentata dai microrganismi patogeni, componenti di un cosmo che muta continuamente e rapidamente e ciò rende ragione dell'obbligatorio mutare della terapia stessa. In questo campo ben poche impostazioni terapeutiche sono rimaste ferme nel tempo, forse nessuna. L'intento è stato quello di mettere a disposizione del medico un manuale di pratica e rapida consultazione che non si limiti a una serie di schemi ma che fornisca, accanto a essi, le basi razionali e le motivazioni dei quotidiani comportamenti terapeutici.
