La presente monografia si prefigge come obiettivo prioritario di mettere a disposizione del lettore, le più recenti conoscenze nel vasto campo pluridisciplinare del carcinoma colorettale, in particolare quelle relative all’epidemiologia, alla genetica, alla chemioprevezione, all’anatomia patologica, alla chirurgia, alla radioterapia, alla chemioterapia, alle terapie biologiche ed alle interazioni varie che si determinano velocemente e che sono in continuo sviluppo. Per l’ampiezza delle problematiche affrontate, la monografia è indirizzata allo studente in medicina che voglia approfondire questo specifico argomento, allo specialista oncologo, al chirurgo, al radioterapista, ma anche all’internista, all’epidemiologo ed al medico di base, cultore dell’argomento. In pochi altri tumori come quelli del colon-retto si sono sviluppate in così poco tempo nuove conoscenze biologiche da una parte e nuove strategie terapeutiche innovative dall’altra. Tutto ciò ha determinato un significativo miglioramento per quanto riguarda la prevenzione, la diagnostica da una parte e le strategie terapeutiche dall’altra (sia nelle fasi iniziali che avanzate della malattia). Infatti, negli ultimi 20-30 anni abbiamo assistito ad una rivoluzione nelle conoscenze sui tumori del grosso intestino, nel campo genetico-molecolare, nella ricerca traslazionale, nella diagnostica per immagini, nell’approccio multimodale e nelle terapie chirurgiche, tant’è, fino agli anni ’80 del secolo scorso una monografia sul carcinoma colorettale avrebbe affrontato quasi esclusivamente le problematiche della chirurgia, ad opera delle innovazioni introdotte da grandi maestri quali Lembert, Miles, Hartman, Goligher, Corman, Valdoni, Knight e Griffen, Heald e Ryall. Oggi il ruolo del chirurgo, benché sempre di primaria importanza, è ricollocato in un ambito multidisciplinare e le decisioni successive all’atto operatorio sono subordinate ad un “team” di esperti, in primo piano l’oncologo, che da angolature diverse valutano il tumore. Questo volume ha anche il pregio di voler sottolineare quanto è complessa ma essenziale l’interazione tra terapie chirurgiche e mediche nell’approccio a questi tumori, anche quando siano in fase metastatica, il che era senza dubbio più difficile da affrontare appena qualche anno fa. Ad esempio, oggi si può arrivare, grazie al supporto della terapia biologica, a rendere operabili pazienti che fino alla fine del secolo scorso sarebbero stati giudicati inoperabili in senso oncologicamente radicale. Sono altresì cadute le barriere discriminanti legate al fattore eta` ed a patologie quali l’infezione da HIV, sicché in queste categorie di pazienti la concomitanza di un carcinoma del colon-retto non è più una controindicazione a trattamenti con finalità curative. Inoltre, va ricordato come altri fattori, quali l’alimentazione e l’attività fisica, giochino un ruolo importante nello scenario di questa importante patologia e su tali fattori bisognerebbe intervenire mediante massicce campagne d’informazione volte ad “educare” la popolazione generale.Non bisogna dimenticare i fattori genetici che frequentemente vengono sottovalutati, tenendo conto che spesso i pazienti che si avvicinano per la prima volta all’oncologo, con una diagnosi di cancro del colon-retto, hanno familiari con lo stesso tipo di tumore e mai indirizzati ad uno screening appropriato. Mentre nel tumore della mammella e nel tumore del collo dell’utero le campagne di prevenzione hanno sensibilizzato la maggioranza della popolazione, per quanto riguarda il cancro del colon-retto in Italia siamo a percentuali inferiori al 10% della popolazione che si sottopone a screening. Fortunatamente abbiamo assistito negli ultimi anni a parecchie iniziative regionali per promuovere lo screening del carcinoma colorettale, ma occorre ancora da fare molto, ed anche in tal senso la nostra monografia vuole offrire un contributo culturale.
