Percutaneous Coronary Intervention in the Patient on Oral Anticoagulation

Andrea Rubboli , Eric Eeckhout
e altri

editore: Oxford University Press

pagine: 96

The management of patients committed to long-term oral anticoagulation, such as those with atrial fibrillation at moderate to high risk of stroke, mechanical heart valves, and previous arterial/venous thromboembolism, who are submitted to percutaneous coronary intervention (PCI) has become an issue of increasing importance in the last years. Guidelines/expert consensus papers addressing the complex management of this population, which is estimated to represent about 5 to 10% of all patients referred for PCI, have been recently made available. As underlined in the most recent guidelines for the management of atrial fibrillation issued by the European Society of Cardiology (ESC) however, "guidelines are no substitutes for textbooks". This pocketbook addresses the key management points and summarises the properties of the stents and the clinical pharmacology of the antithrombotic agents to be used in these patients. It will be a useful guide for clinical and interventional cardiologists, internists, surgeons, primary care doctors, and other physicians who care for these patients.

Oxford Textbook of Interventional Cardiology

Martyn R. Thomas , Nick Curzen
e altri

editore: Oxford University Press

pagine: 752

Cardiac catheterization and coronary angiography remain the key tools in the management of patients with coronary heart disease. Interventional cardiology is now routinely performed outside of major cardiac centres, often in small district hospitals. General training in cardiology rarely offers more than the opportunity to assist a more senior operator to perform angioplasty procedures, so a textbook for the non-specialist has become essential reading. Oxford Textbook of Interventional Cardiology is a definitive text spanning the whole spectrum of interventional cardiology procedures, including management of patients with coronary artery disease, one of the leading killers in western society. Written by an expert faculty of international authors, it offers guidance on all aspects of interventional cardiology according to the European curriculum and guidelines for practice, and covers the very latest techniques and devices to provide practical, evidence-based guidance on treating a full range of coronary lesions. The book also covers structural heart disease and new developments in heart valve disease, which were previously in the realm of cardiac surgeons but are now being treated by cardiologists using less invasive methods.

Heart Valve Surgery

An Illustrated Guide

Jan Dominik , Pavel Zacek

editore: Springer-Verlag Berlin and Heidelberg GmbH & Co. K

pagine: 427

T e four heart valves reside in the center of the heart. T is indicates their crucial role in cardiac performance. Fau- less function of the valves is a prerequisite for unidir- tional forward movement of the blood, and such function is necessary to support the ef orts of the cardiac atria and ventricles. Healthy heart valves function gracefully and of er mechanical durability. Bioengineers have to marvel at the biomechanical evolution of these perfectly placed valves. Heart valves can be involved in pathological processes, however, and only then do we realize just how indispensable they really are. At one time, serious valve disorders used to be a matter of life and death for patients. Only in recent decades have surgeons been able to reverse the ominous course of heart valve disease and of er patients a quality of life and life span comparable to that of healthy persons. T e story of this ef ort began approximately 100 years ago, and today heart valve surgery is a substantial subspecialty of cardiac surgery, with accumulated experience in indications, procedures, risks, and outcomes. T e aim of this book is to present a richly illustrated compendium of the present knowledge related to heart valve surgery, based on the clinical expertise of the authors as well as the newest treatment modalities. T e authors thank Dr. Alireeza Matloobi from the Mayo Clinic for his help in preparating the book.

Aortic Root Surgery

Abraham Charles Yankah , Roland Hetzer
e altri

editore: Steinkopff Darmstadt

pagine: 616

The surgical results of bioprosthetic aortic valve replacement in the 1960s and 1970s were not very satisfactory. The search for the ideal substitute for the diseased aortic valve led Donald Ross to develop the concept of the aortic allograft in 1962 and the pulmonary autograft in 1967 for subcoronary implantation, and later, in 1972, as a full root for replacing the aortic root in the infected aortic valve with a root abscess. The aortic al- graft and pulmonary autograft surgical procedures were revo- tionary in the history of cardiac valve surgery in the last m- lennium because they compete well with the bioprosthesis, are nonthrombogenic (thus, requiring no postoperative anticoa- lation), are resistant to infection, restore the anatomic units of the aortic or pulmonary outflow tract, and offer unimpeded blood flow and excellent hemodynamics, giving patients a b- ter prognosis and quality of life. Surgery for congenital, degenerative, and inflammatory aortic valve and root diseases has now reached a high level of maturity; yet an ideal valve for valve replacement is not available. The- fore, surgeons are focusing their skills and their clinical and s- entific knowledge on optimizing the technical artistry of val- sparing procedures.
