L'aumento della prevalenza del diabete mellito è ormai esponenziale e il suo impatto sulle malattie cardiovascolari risulta sempre più evidente. Il paziente affetto da diabete presenta infatti un rischio notevolmente aumentato di sviluppare eventi cardiovascolari maggiori. Ciò è dovuto alla particolare aggressività della malattia aterosclerotica a livello dei vasi, favorita dalla disfunzione endoteliale legata allo stato iperglicemico. Inoltre, tali alterazioni patologiche risultano proporzionali ai livelli di glicemia e si manifestano quindi anche nel paziente non ancora diabetico, secondo un "continuum" tra glicemia ed eventi cardiovascolari. L'opera illustra, in quest'ottica, gli aspetti fisiopatologici e clinici della malattia cardiovascolare nel paziente con diabete mellito e sindrome metabolica, fornendo un approccio trasversale alla trattazione degli argomenti, dai meccanismi fisiopatologici fino ai risultati dei più recenti trial clinici. Ne deriva una comprensione globale del fenomeno che non include soltanto l'interpretazione del dato clinico ma è frutto di un'attenta analisi dei risultati della ricerca cardiovascolare nel diabete.
