Nell’era della travolgente rivoluzione di Google e dei social, dove ormai anche le conoscenze in campo medico scorrono veloci su supporti digitali, la pubblicazione di un libro cartaceo di diagnostica per immagini sembra un esercizio accademico anacronisticoe insensato. L’Anatomia Radiologica del Dr. Fernando Mazzucato, o familiarmente “il Mazzucato”, nella sua edizione originale ha rappresentato per i radiologi della mia generazione, ma anche per quelli più giovani e per molti tecnici di radiologia, non un semplice libro, uno dei tanti che illustri Colleghi nel corso degli anni hanno donato alle stampe ma “il libro” di tecnica e anatomia radiografica sul qualetutta una generazione di professionisti si è formata e ha mosso i primi passi fra le tante “linee e ombre” della radiologia tradizionale. Un vero punto di riferimento per eseguire e capire la difficile arte della radiologia attraverso la puntigliosa descrizione della tecnica e delle proiezioni, elementi indispensabili per entrare nell’intimo dell’interpretazione dell’anatomia normale, il tutto supportato da memorabili schemi esplicativi che pongono in risalto linee e ombre fino a pochi momenti prima totalmente indiscriminabili. Nelcorso di circa vent’anni “il Mazzucato”, consolidata la sua funzione di testo base, aveva tentato di interpretare e illustrare, con le successive edizioni, anche se con minore efficacia, le nuove metodiche di imaging che avevano via via sostituito nella routine levecchie radiografie. È facile intuire pertanto il mio stupore quando una sera di un paio d’anni fa ricevetti una telefonata: “Sono Mazzucato”; non seppi che rispondere: “ma Mazzucato… Mazzucato, quello del libro?...”. Una telefonata cordiale ma a tratti severa che mi annunciava l’intenzione (credo a malincuore, di mettere mano al “Mazzucato”; di rispolverare e dare nuova luce ad una “creatura”, la sua creatura, frutto di anni di duro, puntiglioso e a tratti esasperato lavoro ma che stava inesorabilmente sbiadendo all’inesorabile passare del tempo. Ero stato scelto, fra tanti, per sua esplicita e garbata esternazione, a seguito di un’accurata selezione, i cui criteri e parametri voglio immaginare (e sperare) siano stati molto diversi e variegati rispetto ai soli bibliometrici come h-Index o IF che purtroppo tutti noi siamo costretti ad adottare nelle procedure di selezione. Ho accettato la collaborazione solo dopo alcuni giorni di riflessione, non tanto per la preoccupazione sulle eventuali difficoltà “tecniche” di lavorare su un libro di radiologia, ma per la consapevolezza che mi sarei trovato nella situazione di un “restauratore”chiamato a dare nuova luce (secondo i dettami della moderna radiologia?) ad un’opera famosa, al severo e intransigente cospetto e giudizio dell’Autore. Ho accettato di lavorare solo sul testo originale della prima edizione dell’Anatomia Radiologica; una sfida difficile ma intrigante dove erano chiari i rischi di un restauro troppo leggero e quindi inutile, o al contrario troppo “invadente” e quindi snaturante l’opera originale. Ho accettato soprattutto perché conscio di avere alle spalle un grande “gruppo” di giovani e meno giovani radiologi e tecnici che ho coinvolto con entusiasmo e con entusiasmo ha risposto mettendo in campo l’energia, l’esperienza, la freschezza necessarie. Da quel momento le occasioni di incontro-scontro con Fernando Mazzucato (mi aveva intanto concesso un confidenziale “tu”) si sono moltiplicate. Due visioni della radiologia diverse, due generazioni a confronto animate però da una profonda e comunepassione per la disciplina. Aveva manifestato più volte la volontà da me sempre accolta, di venire ad Ancona in Istituto per toccare con mano dove e come il suo “restauratore” lavorava e gestiva la “bottega radiologica”. La stima reciproca e la confidenzaaumentavano parallelamente alla consapevolezza del difficile lavoro che stavamo facendo: da una parte il “conservatore”, garante di una tradizione ormai obsoleta, dall’altra l’“ex (N.d.A.) giovane radiologo”, volto all’innovazione e a proprio agio con le nuovetecnologie rispetto alle più datate radiografie.  Il Dr. Mazzucato è scomparso prima di poter vedere la sua nuova opera “restaurata”. Mi rimangono le decine di post-it con commenti e correzioni calligrafi scritti a penna o matita a tutti i capitoli che via via rivedeva appena pronti. Mi rimane il ricordo diun uomo e un Radiologo appassionato che ha amato la professione e la radiologia come pochi.  Il libro oggi prende forma più moderna; l’impostazione più agile e omogenea affronta in maniera sistematica i singoli apparati, entrando nella descrizione pratica delle proiezioni indispensabili per lo studio radiologico delle strutture normali con nuove foto esplicative su come posizionare paziente e tubo radiogeno. Si affianca il risultato iconografico con l’immagine radiografica corredata dalla descrizione anatomica; molti degli schemi anatomici e dei disegni sovrapposti all’immagine originale, che avevano sicuramente caratterizzato e reso celebre la prima edizione del “Mazzucato”, sono stati mantenuti; appaiono ancora attuali perché semplici, razionali, efficaci. Sono state affiancate alle illustrazioni per ciascun apparato nuove tabelle riassuntive sulla tecnica e sulla descrizione delle proiezioni che rendono la lettura e la consultazione del testo più facile e veloce. Il risultato è un manuale pratico con un’impostazione più moderna, ad uso delle nuove generazioni abituate alle diverse forme di “navigazione” informatica. Il testo è stato concepito per gli specializzandi delle discipline radiologiche e per gli studenti della laurea triennale in TSRM; potrà esser un utile ausilio per muovere i primi passi nell’anatomia radiografica con un’ottica più attuale e mediata dalle moderne conoscenze, ma anche un compagno sicuro per rispolverare vecchie nozioni, a volte troppo spesso dimenticate anche dai radiologi e TSRM più anziani. Il nuovo “Mazzucato-Giovagnoni” viene dato alle stampe con una certa emozione, consapevoli di aver operato profondi tagli e modifiche che si spera si rivelino all’altezza dell’edizione originale; il nostro impegno è stato grande, speriamo lo sia il risultato. Un grazie ancora a tutti i giovani radiologi in formazione della nostra Scuola di Specializzazione, un grazie ai colleghi meno giovani del Dipartimento che hanno lavorato con dedizione, consapevoli di aver potuto contribuire alla restaurazione di una opera storica. Un ringraziamento e un ricordo caro al Dr. Fernando Mazzucato che ci ha regalato questa grande opportunità.
