Nowadays, more than 850 million people worldwide suffer from kidney disease. Over the last ten years, new therapeutic options have been discovered for slowing down chronic kidney disease progression. In this book, new multidisciplinary strategies (e.g., related to nephrology, cardiology, primary care, endocrinology, etc.) for treating patients suffering from chronic kidney disease - with and without diabetes - are fully explained and detailed. This book will give the reader the opportunity to delve into the risk factors for chronic kidney disease progression and its multifactorial treatment, to analyze the classical renin-angiotensin system protection, and to examine in depth the new strategies already present on the market for delaying kidney disease progression and decreasing cardiovascular risk in patients at high risk of cardiovascular mortality. Even though in the last decade the future of people with chronic kidney has clearly improved, the implementation of already accepted renoprotective strategies and new therapies is still crucial to prevent kidney disease from progressing to its end stage.

La disciplina di Medicina d’Emergenza e Urgenza comporta un diverso ed unico modo di agire nei confronti di pazienti quasi sempre sconosciuti ed in gran numero che giungono contemporaneamente in Pronto Soccorso con sintomi spesso aspecifici o iniziali e con presentazioni talora insolite di malattie comuni o con sintomi anche classici ma di malattie rare. Per questo motivo più che in ogni altro contesto si è sviluppato il pensiero per algoritmi decisionali, che contengono aspetti diagnostico-terapeutici contemporanei tra loro e sequenziali. Altra caratteristica dell’approccio in Pronto Soccorso, specie per patologie tempo-dipendenti in cui la presa in carico precoce e corretta è fortemente influente la prognosi in termini sia di mortalità sia di esiti permanenti, è il lavorare in team con gli specialisti di altre discipline e con le altre professioni sanitarie, con il medico d’emergenza e urgenza a fungere da team leader coagulando intorno a sé gli apporti tecnici e culturali degli altri operatori con il fine di garantire rapidamente il miglior percorso al paziente affetto da uno stato morboso evidentemente pericoloso per la vita. Questa organizzazione virtuosa non conduce solamente al miglioramento della prognosi dei pazienti, ma anche e soprattutto alla loro identificazione corretta nei numeri e precoce nei tempi, essendo spesso il Pronto Soccorso la sede di primo contatto medico con una grande varietà di patologie anche rare, tra le quali le Microangiopatie Trombotiche, alcune forme delle quali presentano alta mortalità ed esiti anche gravi se non correttamente diagnosticate e trattate all’esordio, e la cui sottostima attuale risulta chiara a chiunque abbia una certa esperienza di Pronto Soccorso. È quindi all’approfondimento di queste patologie che, in maniera multidisciplinare ed orientata al setting dell’emergenza e urgenza, è dedicato questo volume, frutto del lavoro di esperti riconosciuti delle diverse discipline coinvolte nel percorso diagnostico terapeutico assistenziale delle Microangiopatie Trombotiche.
