PrefazioneLa 14esima edizione delle Linee Guida è stata scritta nel corso del verificarsi di una circostanza straordinaria: la pandemia da coronavirus. Questo fenomeno globale ha modificato radicalmente le vite e le pratiche lavorative di miliardi di persone e la maggior parte di noi, oggi, ha conosciuto, personalmente o indirettamente, la grave malattia fisica associata al COVID-19. Ad esserne particolarmente colpiti sono stati gli operatori sanitari che si trovano ad essere a contatto con quanti si sono ammalati, rischiando essi stessi di contrarre l’infezione. In un simile contesto, la stesura di un libro ha una priorità estremamente bassa, se non addirittura nulla. È tenendo conto di ciò che rivolgo il mio più vivo e sincero ringraziamento a tutti coloro che hanno contribuito alla realizzazione di questa edizione delle Linee Guida in condizioni così difficili. Naturalmente, durante la pandemia, i problemi di salute mentale non sono scomparsi e il trattamento ottimale delle malattie mentali rimane un imperativo vitale. Tale obiettivo diventa ancora più importante quando affrontiamo le conseguenze della pandemia. Questa edizione delle Linee Guida è stata accuratamente aggiornata ed include le ricerche più influenti pubblicate a partire dal 2017 e tutti i principali farmaci psicotropi che sono stati da allora introdotti. Questa edizione prevede inoltre nuove sezioni dedicate a temi quali il delirium con agitazione, l’impiego dei farmaci psicotropi nei pazienti terminali, le formulazioni dei farmaci psicotropi da somministrare per via endovenosa, la somministrazione di clozapina per via intramuscolare e la somministrazione settimanale di penfluridolo per via orale. Come le precedenti, questa 14esima edizione è stata realizzata con l’intento di avere un’utilità a livello mondiale, pur avendo una certa influenza soprattutto nel Regno Unito. Vorrei rivolgere un ringraziamento particolare a Siobhan Gee per il meticoloso lavoro compiuto sull’impiego della clozapina, a Mark Horowitz per le indicazioni basate sull’evidenza e incentrate sul paziente in relazione alla cessazione della somministrazione dei farmaci psicotropi, a Delia Bishara, la quale si è occupata praticamente da sola del capitolo dedicato agli anziani e a Ian Osborne per il contributo fornito in relazione ad una gamma eccezionalmente ampia di temi. Un riconoscimento particolare merita Emily Finch per l’organizzazione della stesura del capitolo sulle dipendenze nelle ultime dieci edizioni del le Linee Guida. Infine voglio porgere il mio ringraziamento alla mia assistente Ivana Clark, che ha contribuito alla realizzazione di questa edizione con pazienza e incomparabile attenzione al dettaglio.
