PrefazionePer i medici, l’ecografia è uno strumento prezioso per visualizzare in tempo reale determinate parti del corpo umano, ma richiede un addestramento significativo. Proprio per questo, abbiamo iniziato a insegnare agli studenti di medicina della St. George’s University l’anatomia ecografica pratica durante la loro formazione di base. Crediamo che l’uso crescente dell’ecografia in varie specialità mediche nell’ultimo decennio richieda una solida base nella tecnica ecografica, nell’anatomia e nella fisiologia fin dall’inizio della formazione medica. L’ecografia al letto del paziente (point-of-care ultrasound) è ora ampiamente disponibile in cliniche e ospedali, rendendo le competenze nell’esame ecografico cruciali quanto l’uso di uno stetoscopio. Per molti, l’ecografia è lo stetoscopio del futuro.Una delle sfide che abbiamo affrontato nelle prime fasi dell’implementazione dell’ecografia nel curriculum è stata come ottimizzare il tempo che gli studenti trascorrevano con i pazienti durante le sessioni pratiche. Ci siamo resi conto che troppo tempo veniva speso nella parte teorica per spiegare il posizionamento del paziente, il posizionamento della sonda e l’aspetto delle immagini ecografiche. L’obiettivo era che gli studenti arrivassero preparati alle sessioni pratiche con una comprensione di base del corretto posizionamento del paziente, del posizionamento della sonda e di come identificare i principali punti di repere nelle immagini ecografiche in tempo reale. Pertanto abbiamo perseguito l’obiettivo di fornire materiale teorico di riferimento che consentisse agli studenti di arrivare al laboratorio pronti per iniziare ad esercitarsi con la sonda ecografica, con la guida e l’assistenza dei docenti.Le risorse didattiche esistenti sulle tecniche ecografiche si rivolgono principalmente ai medici che già utilizzano l’ecografia nella loro pratica clinica. Queste risorse si concentrano soprattutto sugli aspetti tecnici delle applicazioni cliniche. Il nostro libro mira a colmare questa lacuna offrendo una base nell’anatomia ecografica topografica accessibile agli studenti nelle fasi iniziali della loro formazione medica e sufficientemente dettagliata da interessare anche i medici che integrano già l’ecografia nella loro pratica quotidiana. Il libro è organizzato per regioni anatomiche, e ogni sezione segue una struttura simile. Inizia con una panoramica dell’anatomia rilevante, seguita da una sezione sulla tecnica. Entrambe le sezioni forniscono spiegazioni testuali e illustrazioni riguardanti i dettagli tecnici e le abilità necessarie per ottenere specifiche immagini ecografiche. Ogni figura del libro include un’immagine ecografica di base e una con tutte le strutture contrassegnate, una rappresentazione della struttura dei tessuti, un disegno che illustra il posizionamento del paziente e della sonda e una sezione di cadavere con tutte le strutture contrassegnate corrispondente alle immagini ecografiche. La spiegazione della tecnica è accompagnata da schede di riferimento rapido che identificano l’orientamento dell’immagine ecografica, il contenuto e rimandano alla figura specifica in cui si trova l’immagine.Il libro può essere letto capitolo per capitolo o sezione per sezione per ottenere una panoramica delle immagini ecografiche e delle strutture importanti incontrate in ciascuna regione anatomica. In alternativa, i lettori possono facilmente individuare immagini ecografiche specifiche insieme a illustrazioni e descrizioni di come ottenere tali immagini e identificare le strutture e i punti di repere al loro interno. I docenti possono selezionare due o tre immagini da una regione anatomica e assegnare le letture corrispondenti per preparare gli studenti alle sessioni di laboratorio, dove praticheranno il corretto posizionamento del paziente, il posizionamento della sonda e l’identificazione dei punti di repere e delle strutture.In questa seconda edizione, abbiamo apportato miglioramenti significativi per rendere il libro ancora più utile per la futura pratica clinica. Quasi ogni capitolo è stato ampliato con contenuti aggiuntivi. Inoltre, le dissezioni cadaveriche sono state migliorate per fornire una visione più chiara dell’intera area dissezionata, sostituendo le immagini poco chiare delle edizioni precedenti.Abbiamo anche ampliato le domande a scelta multipla per l’USMLE (United States Medical Licensing Examination), che ora sono incluse in ogni capitolo.
